1-2 Violation of Code: [.03(3)(a)-(d) ] Failure to post food safety certification in public view. No food safety certification is posted in public view of the facility. The certified food safety manager was not here at the time of inspection and his certificate could not be posted. The person in charge was advised to obtain and post in public view an original full size copy of the food safety certificate. New Violation.
2-1C Violation of Code: [.04(4)(a) ] Bare hand contact with ready-to-eat foods. Employee observed touching cut and washed bell peppers with bare hands. The bell peppers were discarded and the person in charge was advised that all ready to eat foods should only be touched with clean gloved hands to help prevent possible contamination from bare hands. Corrected On-Site. New Violation.
2-1C Violation of Code: [.04(4)(a) ] The sushi chef was observed cutting and touching cut cucumbers with bare hands. The cucumber was discarded and the employee washed hands and put on gloves before continuing to cut the cucumbers. All ready to eat foods should only be touched with clean gloved hands to help prevent possible contamination from bare hands. Corrected On-Site. New Violation.
4-1A Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] Raw squid above stored above sauces in walk in cooler, raw chicken stored above beef in the walk in cooler, raw chicken stored above sauces in the prep cooler, raw chicken removed from original packaging stored above cream cheese wontons in the freezer. The person in charge was given a hand out on the proper storage of items in coolers. All items were removed and stored properly according to internal cook temperatures. Corrected On-Site. 4-2B Violation of Code: [.05(7)(a)1 ] Two knives and a serving spoon found stored as clean with buildup of food debris present. The person in charge removed these items to be properly cleaned to sight and touch. All food contact surfaces should be stored clean to sight and touch to help prevent possible contamination. Corrected On-Site. New Violation.
5-2 Violation of Code: [.04(7)(e) ] The consumer advisor for the sushi bar is not adequate. An advisory statement is posted on the bar but the sushi items are not labeled or identified in any way to identify which items are raw or undercooked. Proper consumer advisory methods were discussed with the person in charge. New Violation.
6-1A Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. Several items in the walk in cooler such as eggs, cream cheese, and cream cheese mix were found to be above 41 degrees. The person in charge discarded these items and turned down the cooler. All potentially hazardous food items must be held at 41 degrees and below to help prevent possible contamination. Corrected On-Site. Repeat Violation.
6-1C Violation of Code: [.04(6)(d) ] Cooked potentially hazardous foods not cooled from 135oF to 41oF within a total of 6 hours. Observed two large containers of noodles cooling since 12-20 in the walk in cooler were found to be above 41 degrees. The person in charge discarded these containers and was advised to ensure all foods are cooled from 135 to 41 degrees within 6 hours. The person in charge was advised on proper cooling methods. Corrected On-Site. Repeat Violation.
6-1D Violation of Code: [.04(6)(i) ] Container of sushi rice did not have any documentation for the time as a public health control. Time controls not documented according to written procedure. Need to make sure that once the rice get below 135oF you put the time that the rice is taken off of temperature control. The person in charge placed a sticker on this item to indicate the time it was placed on time control. The person in charge was given a copy of the time as a public health control written procedure outline to prepare and keep at the facility. Corrected On-Site. Repeat Violation.
6-2 Violation of Code: [.04(6)(g) ] Refrigerated, ready-to-eat, potentially hazardous food prepared and held in the establishment for more than 24 hours not clearly marked to indicate the date by which the food must be consumed, sold, or discarded. Several pans of mac and cheese in the walk in cooler and two containers of cooked chicken in the sliding cooler that were prepared over 24 hours prior to inspection were not properly date marked. The person in charge date marked these items and was advised on proper date marking procedures. Corrected On-Site. New Violation.
10D Violation of Code: [.04(4)(d) ] Three containers of salt, sugar, and msg at the grill station were found to be stored in working containers that were not labeled with the common name of the food items. All items removed from original packages should be identified with a label of the common name on all working containers. The person in charge properly labeled these items. Corrected On-Site. New Violation.
11C Violation of Code: [.04(6)(c)] Potentially hazardous foods not thawed under an approved method . Raw fish found thawing in the prep sink in standing water at room temperature. The person in charge continued to thaw the fish under cool running water. An approved thawing method should be used when thawing potentially hazardous food items to help prevent possible contamination (see code page 65 for proper thawing methods). Corrected On-Site. Violation of Code: [.04(4)(q) ] Food not stored at least 6 inches above the floor. Several buckets of sauces were found being stored directly on the floor of the walk in cooler. The person in charge removed these items for proper storage at least 6 inches off the floor to help prevent possible contamination. Corrected On-Site. New Violation.
Violation of Code: [.04(4)(u) ] Food not shielded properly on counters, serving lines, salad bars, or other food display. The ice cream and pie cooler at the dessert bar are not properly protected with a proper sneeze guard. Several customers were observed obtaining food from these coolers that are not protected. The person in charge had an employee stand at the dessert area and obtain the ice cream and pies for the customers. All food at the buffet for consumer self service must have proper shielding to help prevent contamination. New Violation.
14A Violation of Code: [.04(4)(k) ] While not in use, food preparation/dispensing utensils not stored with their handles above the top of the food and the container. Utensil in the flour was found with the handle stored down into the food item. The person in charge removed the utensil to be stored properly with the handles above the surface of the food to help prevent barehand contact. Corrected On-Site. New Violation.
